Administrative Fees & Documentation Policy

At Balanced Psychiatry, we understand that you may occasionally require clinical documentation, school/work accommodation forms, or formal letters to support your care. Because reviewing comprehensive histories and drafting custom medical documentation requires dedicated clinical time outside of your face-to-face appointments, these requests are billed as standalone administrative services.

Fee Schedule for Letters & Paperwork

  • Basic Documentation ($75): Standard return-to-work/school clearance notes, jury duty medical excuses, or basic medication verification letters.
  • Complex Academic/Workplace Accommodations ($150): Detailed ADHD academic accommodation forms (504 Plans or IEP documentation) and complex employer wellness or scheduling accommodations.
  • High-Liability Documentation ($200): Emotional Support Animal (ESA) letters, Family and Medical Leave Act (FMLA) paperwork, or short-term disability documentation.

Our Policy Boundaries

  • The Established Relationship Rule: To maintain the highest clinical and legal integrity, we do not provide high-liability documentation (such as ESA or disability letters) during an initial evaluation. These requests will only be considered for patients who have been actively engaged in an established treatment relationship with Balanced Psychiatry for a minimum of 30 to 90 days.
  • Processing Window: All paperwork requests require a minimum of 5 to 7 business days to review and complete. We kindly ask that you submit your requests well in advance of your deadlines.
  • Document Release: Completed documents will be securely uploaded directly to your patient portal once the administrative invoice is settled.
